The practice known as Eramosa Family Medical Centre will be transferred to a new health service provider on or about 12 December 2025.

Dr Shaun Zail will continue to practice at Eramosa Family Medical Centre for the foreseeable future, and from a patient’s perspective there will be no noticeable change.

All health information held by Eramosa Family Medical Centre regarding patients will remain in the secure practice management software for continued access by the doctors.

If any patient would prefer not to have their records maintained at the practice, they will be available for collection at the practice or to be sent to the individual or the individual’s nominated health service provider from the date of this notice until on or about 12 December 2025.
